Phone number and puk code?

Please let me know my phone number and my sim card puk code? My cousin brother sent to me My new iphone 6 plus device from Dubai,UAE. At&t sim card already had.but my cousin brother could not know phone number.he not fill up sim card.he thaight that is already package with at&t sim. So that how can i know my sim number and puk code( my sim is already locked) ps. My country is Myanmar as known as Burma.
